These Terms and Conditions ("Terms") govern your use of the CARED Patient Record Portal ("Platform", "Service", "CARED") operated by PatientRecordPortal under the direction of Dr. Sunil Kurakula.

By registering for an account, accessing, or using any part of the Platform, you acknowledge that you have read, understood, and agree to be bound by these Terms.

2. Service Description

CARED is a patient-centered digital platform designed to help individuals:

• Organize and store personal health records
• Create summaries of health information
• Share health records with healthcare providers
• Access educational health resources
• Track medications and allergies

3. User Eligibility

To use CARED, you must:

• Be at least 18 years of age, OR
• Have explicit parental or legal guardian consent if under 18
• Have the legal capacity to enter into binding contracts
• Provide accurate and complete registration information
• Maintain the security and confidentiality of your account credentials

4. User Responsibilities

4.1 Account Security

You are responsible for:

• Maintaining the confidentiality of your password and account
• All activities that occur under your account
• Notifying us immediately of any unauthorized access
• Ensuring your contact information remains current and accurate

4.2 Data Accuracy

You agree to:

• Provide accurate, complete, and current health information
• Update your records promptly when information changes
• Verify the accuracy of uploaded documents
• Ensure all shared information is appropriate and relevant

4.3 Prohibited Uses

You agree NOT to:

• Use the Platform for any illegal purposes
• Upload false, misleading, or fraudulent information
• Share your account credentials with unauthorized persons
• Attempt to gain unauthorized access to the Platform or other users' data
• Use the Platform to distribute malware, viruses, or harmful code
• Engage in any activity that disrupts or interferes with the Platform
• Copy, modify, or create derivative works of the Platform
• Use automated systems (bots, scrapers) to access the Platform
• Reverse engineer or attempt to extract source code
• Share health information without explicit consent from the data subject

5. Premium Subscription Services

5.1 Subscription Plans

CARED may offer premium subscription services including:

• Personalized health coaching
• Advanced AI-powered insights
• Priority customer support

5.2 Payment Terms

• Subscription fees are billed in advance on a monthly or annual basis
• Payments are processed through secure third-party payment processors
• Prices are subject to change with 30 days' notice
• You authorize us to charge your payment method for recurring subscriptions

5.3 Refund Policy

• 7-Day Money-Back Guarantee: Full refund if cancelled within 7 days of initial subscription
• Monthly Subscriptions: Pro-rated refunds for mid-cycle cancellations
• Annual Subscriptions: No refunds after 30 days
• Refunds are processed within 10-15 business days

5.4 Cancellation

You may cancel your subscription at any time through your account settings. Cancellation takes effect at the end of the current billing cycle.

6. Intellectual Property Rights

All content, features, and functionality of CARED, including but not limited to:

• Software, code, and technology
• Text, graphics, logos, and images
• Design, layout, and user interface
• Educational content and resources

are owned by PatientRecordPortal and protected by copyright, trademark, and other intellectual property laws.

6.2 Your Data

You retain all ownership rights to your health records and personal data uploaded to CARED. By using the Platform, you grant us a limited license to:

• Store and process your data to provide our services
• Create summaries and visualizations of your health information
• Use anonymized, aggregated data for platform improvements (with your consent)

7. Fair Use Policy

CARED is intended for:

• Personal Use: Managing your own or dependents' health records
• Record Organization: Storing and summarizing medical documents
• Education: Learning about health conditions and prevention

NOT for:

• Emergency medical diagnosis or treatment
• Mass data collection or commercial use
• Sharing others' data without explicit consent
• Professional medical record-keeping by healthcare institutions

8. Limitation of Liability

To the maximum extent permitted by law:

CARED does not provide medical advice, diagnosis, or treatment. All educational content is for informational purposes only. Always seek the advice of qualified healthcare professionals.

8.2 Platform Availability

We do not guarantee that:

• The Platform will be available at all times without interruption
• All errors or defects will be corrected
• The Platform will be free from viruses or harmful components
• Data backups will always be successful

8.3 Liability Limits

PatientRecordPortal shall not be liable for:

• Indirect, incidental, or consequential damages
• Loss of data, profits, or business opportunities
• Personal injury or property damage
• Medical decisions made based on Platform data
• Third-party actions or content

Our total liability shall not exceed the amount paid by you for subscription services in the 12 months preceding the claim.

9. Indemnification

You agree to indemnify and hold harmless PatientRecordPortal, its directors, employees, and agents from any claims, damages, losses, or expenses (including legal fees) arising from:

• Your violation of these Terms
• Your use or misuse of the Platform
• Your violation of any third-party rights
• Any false or inaccurate information you provide

10. Termination

10.1 By You

You may terminate your account at any time by:

• Requesting account deletion via email to [email protected]
• Using the account deletion feature in your settings

10.2 By Us

We may suspend or terminate your account if you:

• Violate these Terms
• Engage in fraudulent or illegal activities
• Compromise the security or integrity of the Platform
• Fail to pay subscription fees

10.3 Effect of Termination

• Your right to access the Platform will immediately cease
• You may request a copy of your data within 30 days
• Data will be deleted after 90 days as per our Privacy Policy
• No refunds will be provided for terminated accounts

12. Governing Law and Jurisdiction

13. Changes to Terms

14. Severability
